Blackout Hospital Official Walkthrough!


This is a long one everyone! Be Prepared!

This is the Blackout Hospital Official Walkthrough, made by me, arcadekitten! If you follow the steps in this guide and things are still not working according to it/you are unable to continue, then something is wrong and I need to re-check it to make sure it works as intended. Please report any bugs to me.

This game is separated into unofficial chapters, with each chapter “ending” around after their respective boss fight. This is to help organize the game as well as the walkthrough and any bug reports to me.

(ALSO, LIKE, SPOILERS 'N STUFF I GUESS. BE WARNED.)

Chapter 1.

This starts as soon as the game begins. To continue forward:

  • Clean up the spills on the 5th floor of the hospital. You must be standing ABOVE the spills to clean them, not in front of.
  • After speaking with Rune in the elevator, clean the puddle in front of Room 605.
  • The lights have gone out. Travel down to the 4th Floor and inspect the elevator. There should be a puzzle-lock on it. 
  • Travel to the 5th floor. Inspect the doctor’s coat hanging near the door to receive 5 MONEY.
  •  Bring the money to the vending machine on the 5th floor. Purchase a TEA to bring down to the elevator. 
  • Fight the miniboss inside the elevator.
  • After defeating it, Vasilis will attempt to leave through the entrance doors to no avail. Head back down to the bottom of the screen, Vasilis will see a shadowy figure drop a key and run into the elevator. Vasilis will have to take the stairs to progress since the elevator is in use.
  • The key is for Room 304. (You can look at the keys in your inventory to see where they belong. ) Enter the room and receive the crayons from the crayon box. Color the eyes of the cat toy blue and then red. 
  • Open the puzzlebox Vasilis receives from under the bed. The buttons must be pushed in color order to open it. RED-ORANGE-YELLOW-GREEN-BLUE-PURPLE. 
  • Take the key and head to the medicine room in the center of the 5th floor. 
  • Inspect the cabinet that has a twinkle on it. Collect the keys the mouse drops. Inspect room 605 first, because Vasilis won’t let you look in room 604 until then. 
  • Read Mary’s diary. You cannot leave the room without doing so. 
  • Leave Mary’s room, and you will run into the shadowy figure again in the hallway. After they join your party, head into room 604. 
  • Clean the bloodstains on the floor until you can find the message and holes. This starts the block collection quest.
  • A drawer in room 604 gives you the key to room 507. Enter that room. 
  • The party encounters Gus. He is incredibly hungry. You must bring him a meal from the cafeteria. 
  • Head down to the cafeteria. It is on the first floor and towards the main entrance, to your left. The hallway to get there is marked with an orange food sign. 
  • Continue past the courtyard in the connecting hall until you reach the cafeteria, and then enter the kitchen. 
  • Now that you are in the kitchen, you can start prepping the meal. Check every cabinet and fridge until you have all the ingredients. (Buns, Lettuce, Cheese, Patty, Ketchup) Head to the cutting board to prepare them.
  • There is one last ingredient for the burger to be complete. You must “type” in the answer. the answer is MARSHMALLOWS. (or singular MARSHMALLOW)
  • Now that the burger is finished, deliver it to Gus. He will give you the block and you will also receive a key to room 308. 
  • Enter room 308 and receive the next item. Then, talk to the birds until they allow you to leave. 
  • When heading back towards the first floor, Mary will stop you to use the bathroom. While waiting, the door to room 303 will open. Enter it. 
  • Encounter Ollie. This is where the “Ollie’s friends” sidequest begins. I will touch on this more at the end of the walkthrough. 
  • Leave the room and return to the 3rd floor. Knock on the bathroom door to check on Mary. After the cutscene passes, head down to the lounge.
  • Enter the lounge and inspect the paper left on the table. Receive the final block. When you attempt to leave the room, you will encounter the first boss. 
  • Defeat the first boss and return to room 604 to put the blocks into their proper holes. 
  • After that, return to room 304 to receive the cat toy. Bring the cat toy back to room 604 and interact with the blocks again.
  • After the cutscene passes, collect the item left behind and head up using the stairs to the 7th floor. You will be able to pass the goo by interacting with it. 

Chapter 2.

  • On the 7th floor is 4 doors with glowing windows. All can be entered except one of them. One of them takes you to a flowershop, one to a restaurant, and one to an outside picnic. 
  • You can enter the areas in any order any time you want, but to make progress, you will have to do things in a certain order. 
  • First, enter the restaurant and talk to the waiter making rounds. They will ask you to help them with carrying out orders. Bring the orders from table to table until one of them gives you a cash tip.
  • Bring your new cash to the flowershop. You don’t have enough money to buy a bouquet, but Drekaride will feel bad for you and give you a rose.
  • Bring the rose to the picnic and place it in the empty vase. You can now pick up the pitcher of water.
  • Bring the pitcher of water to the restaurant and refill Drekaride’s drink. They will be so thankful that they will give you a very big tip. 
  • Bring the tip to the flower shop and collect the bouquet. Go to the last door. 
  • After inspecting and then leaving, head down to the courtyard on the 1st floor and attempt to open the door. The party will hear noise coming from the cafeteria. 
  • Enter the cafeteria and meet Hyllindrix. Talk to him and Mary will trade the ring for the key to the courtyard.
  • Enter the courtyard and place down the bouquet in the center of the bench. You will then have another boss battle.
  • After the boss battle concludes, head back to the cafeteria and confront Hyllindrix. He will now challenge you to a game. Sit in the empty seat across from him to play. 
  • Vasilis can play against Hyllindrix 3 times before the event starts repeating. To escape the loop, ask Mary to sit in the empty chair behind Hyllindrix to cheat the game. 
  • After defeating Hyllindrix, he’ll give you the ring back. You can now also gamble with Hyllindrix any time you like, for 30 money.
  • Return the ring to Minspella, and then after the cutscene finishes, collect the 601 key from Drekaride. 

Chapter 3.

  • Head up to the 6th floor. The elevator should be in use, so use the stairs. 
  • After reaching the 6th floor, Dr. Tetra will join your party. Enter into the shop that just opened up and talk to the shopkeep/nurse.
  • With your new basement key, head down to the basement on the first floor. There should be an elevator with barnacles and a water puddle in front of it. 
  • Enter the basement. 
  • Inspect the room full of electrical panels. Go to the open one and receive the storage room key.
  • Collect the tag on the floor to leave the room.
  • Find the storage room. It should be the right-most room on the second map.
  • Collect the laundry room key and spare room key in that room. 
  • The laundry room is the left-most room on the second map. The spare room is the other room on the first map.
  • In the spare room, find the one open wall on the map. Walk up and through it until you reach a chest. The chest needs a code to be opened. The correct number code is 4520. Collect one of the morgue keys. 
  • In the laundry room, open the shaking washing machine and chase the mouse until you catch it. If you find it difficult, you can corner the mouse behind Dr. Tetra and Mary. 
  • Once the mouse is caught, you can release it to gain a point for Mary or kill it to gain a point for Dr. Tetra. 
  • Use the morgue keys you collected to open the morgue. It should be the last door left.
  • Open the shaking locker and meet Emerald. After she retrieves gloves for you, go back to the electrical panel.
  • You must flick the switches so that the pictures beside them tell a story of an octopus bringing a ship underwater and strangling the sailor. The order of events is

  • A bridge has been made over the chasm. Cross it. 
  • Complete the maze. Enemies lurk in the maze, but you can run from them if you do not wish to fight them. A map of the maze has been provided below. Note that the maze has the possibility to loop horizontally.
  • From the start of the maze, to get to the end, go left, then straight up. Go one step left and then go straight up again. Go right, up, then right again. Up, left, up, right, up, left.
  • At the end of the maze, you will face the third boss fight. 
  • After defeating the boss, you can leave the basement through the stairwell. 

Chapter 4.

  • After leaving the basement, Mary will start having a seizure. You can decide how you're going to help her.
  • Continue to the 7th floor, and head to the bottom of it to get to the bridge
  • After the cutscene with Rune, continue to the elevator and to the 6th floor. Head to the center doors and then attack the goos. The goo that has the key is random, but because Rune has an attack that kills all enemies at once, you should be fine. 
  • Collect the key and enter the double doors. Rune will leave your party.
  • Head to the first floor and towards the direction of the ambulance bay. It will be outlined by a gooey pink path. 
  • Inspect the goo in front of the ambulance bay, then head to the bottom left and see the open door. Collect the X-Room key.
  • Read the red note on the corkboard, and then go to the front desk in the room. There should be a note there now. Read the note, and then head up to the vending machine on the 6th floor. Interact with it and receive Gigi’s janitor key. 
  • Go to the 5th floor and unlock the janitor closet in the center. Approach the toolbox and receive the screwdriver. 
  • Go back to the x-ray room and remove the skeleton’s hands. 
  • Collect the x-ray cassettes from the shelf if you haven’t already.  Have Dr. Tetra man the X-Ray machine and go back and forth x-raying the skeleton’s hands until complete. Mary will talk to you during this. It's up to you how you answer her questions about Dr. Tetra.
  • Insert the cassettes into the working computer. You will get steps on how to move ahead. Stand in front of the skeleton and go down, down, right, down.
  • Collect the OR key and head to the OR room. Another boss battle ensues. 
  • After defeating the boss, collect the collar dropped outside of the OR Room. Head to the 2nd floor through the now-open staircase. 
  • The 2nd floor is covered in slippery goo you must slide across to get past.
  • From the start of the room, go down, right, up, left, up, right, up, left, down, right, up.
  • After entering the room and talking with Lucky, exit the room and follow and talk to Lucky at each place he stops. Once he’s in front of the ambulance bay, he will open the path for you.
  • Enter the ambulance bay. Inspect every garage door. After Vasilis freaks out, collect the key to their supply closet. 

Chapter 5.

  • Go back to the main building, and go into Vasilis’s closet on the 6th floor. Collect the key to the roof.
  • Approach the roof and inspect the strange gate in front of it.
  • Go back to the cafeteria and talk to Hyllindrix. He’ll make you an offer to trade you for cards to unlock the gate. To get items to trade Hyllindrix, you have to collect items from the bosses you’ve defeated in the game. You can bring the items to him one at a time or once you’ve collected them all, but you can only GIVE them to him one at a time. (So you have to interact with him multiple times with the items in your inventory).  
  • Talk to Mallory in the cafeteria. She’ll tell her you can have her puzzlebox. Go to room 304 and collect it. 
  • After leaving Mallory’s room Mary will pull you into the bathroom. Choose how you'll respond to her.
  • Talk to Minspella. She will tell you you can take her ringbox from when Drekaride proposed. You can find it back at the picnic scene on the 7th floor, in the basket.
  • Bring the ringbox to Hyllindrix. He will reject it as he considers it not tradeworthy and force you to get something else. Return to Minspella and she will give you her ring. 
  • Head back to the basement (only accessible by stairs) and talk to Griindel. He will ask you if you can find a necklace he lost in the maze. Dr. Tetra will leave your party for this quest, and you cannot leave the basement once you begin it. 
  • Find Griindel’s necklace in one of the open square areas of the maze and return it to him. Dr. Tetra will rejoin your party. Griindel will take the picture inside the necklace and give you the necklace itself to return to Hyllindrix. 
  • Talk to Lucky in the first floor of the West Building. You will have to cross the bridge on the 7th floor to get there. Talk to Lucky and he will lead you to the door of the X-Ray room.
  • Go inside and remove the skeleton’s leg to bring to Lucky. Once you exit the room, he will take the leg and run–leaving you with the charm from his collar to bring to Hyllindrix. 
  • Once you have 4 ace cards from Hyllindrix, you can bring them all to the green gate and unlock the door. 

ONCE YOU GO TO THE ROOF, DR. TETRA AND MARY WILL DISBAND FROM YOUR PARTY. Certain interactions can only be had when they are in or outside of your party. 

Endings and instructions on how to do certain things to achieve them are listed below. Please be sure to read thoroughly. 

Endings:

There are 3 different endings in this game. A bad, default ending / A secret ending / A true ending. 

To get the bad ending, do what Dr. Tetra told you to do without investigating anything else. 

To get the secret ending, go into Dr. Tetra’s office and make a discovery, or talk to the NPC Ioro in the Shop once you are alone and have completed Ollie’s sidequest. Then, continue with Dr. Tetra’s request.

To get the true ending, go into Dr. Tetra’s office and make a discovery, or talk to the NPC Ioro in the Shop once you are alone and have completed Ollie’s sidequest. Then, go to Mary on the roof and talk to her. 

TO GET INTO DR. TETRA’S OFFICE:

  • To get into Dr. Tetra’s office, you need his key. You can receive this key by being close friends with Mary or Dr. Tetra, but not both. 

  To be close friends with Mary, you need at least 8 of the following points, and to be closer with her than Dr. Tetra: 

+1 for apologizing in room 604

+1 for talking to Mary in the kitchen during Gus’s quest

+1 for letting the mouse go in the laundry room

+1 for inspecting the GameGal in the basement maze

+1 for staying  with her during her seizure

+1 for the X-Ray discussion, tell Mary you like Dr. Tetra

+1 for telling Mary her eye is leaking

+ If you talk to Mary while she’s in your party through the “talk” menu, you can gain 5 extra points for talking at least 5 times, but no more points will be awarded for exceeding 5 times. 

If these qualifications are met, Mary will give you the key to his office automatically. 

To be close friends with Dr. Tetra, you need at least 7 of the following points, and to be closer with him than Mary: 

+1 for killing the mouse in the laundry room

+1 for letting Dr. Tetra put out a lit candle in the basement maze

+1 for getting Mary medicine during her seizure

+1 for X-Ray discussion, tell Mary you TRUST Dr. Tetra

+1 for lying to Mary and telling her that her eye looks fine

+ If you talk to Dr. Tetra while he’s in your party through the “talk” menu, you can gain 5 extra points for talking at least 5 times, but no more points will be awarded for exceeding 5 times. 

If these qualifications are met, you can talk to Dr. Tetra on the stairwell and keep prodding him until he relents the key of his office to you. 

TO COMPLETE OLLIE’S SIDEQUEST AND TALK TO IORO:

NOTE: OLLIE’S SIDEQUEST CAN ONLY BE COMPLETED WITH MARY AND DR. TETRA STILL IN YOUR PARTY. IF YOU ATTEMPT TO TALK TO IORO WHEN YOU ARE ALONE, AND BOTH MITZI AND OLLIE ARE ABSENT, IORO WILL WALK AWAY.

If you do not have enough friendship points with Mary or Dr. Tetra, it is still possible to achieve the true ending, though you will miss whatever is in Dr. Tetra’s office. 

Before heading to the roof, you must find both Mitzi and Ioro, and then Ollie. Mitzi and Ioro can be found in any order, but Ollie will only appear once you have found both Mitzi and Ioro. When you find and talk to them, they will move to stay in the shop. 

Mitzi can be found in the kitchen, but only after Ollie’s quest has been activated when you first meet Ollie. 

Ioro can be found in the basement maze.  

After finding those two, Ollie can be found on the 7th Floor of the West Building (but not if Rune is in your party). Cross the bridge and they will be sitting on one of the chairs. Talk to them and they will go to the shop.

Then once YOU ARE ALONE, go back to the shop and talk to Ioro. He will provide you with information that renders you capable of unlocking the secret and/or true ending.
